EHCP tracking software

EHCP tracking software that keeps outcomes at the centre

Track every EHCP outcome term by term, see progress calculate from the pupil’s learning plan, and pull a year’s evidence straight into the annual review — without rebuilding it in Word.

Why EHCP tracking matters

An Education, Health and Care plan sets out the outcomes a young person is working towards and the provision they need to get there. The SEND Code of Practice requires the plan to be reviewed at least every 12 months, and schools are expected to show clearly how the young person is progressing towards each outcome.

When outcomes, targets and evidence sit in different documents, review preparation becomes a scramble. EHCP tracking software keeps them together all year.

How ASEND360 tracks EHCP outcomes

  • EHCP Tracker: each outcome rated termly as met, partly met or not yet met.
  • Linked to the ILP: outcomes flow into Individual Learning Plans as small, measurable steps, and outcome ratings calculate from those steps.
  • Provision and interventions linked to outcomes, so you can show what support was put in place and how often it was delivered.
  • Optional Goal Attainment Scaling with a trend line for each target.

Annual reviews without the paperwork chase

ASEND360 drafts the school’s contribution from the EHCP Tracker and ILP for you to edit, keeps professional advice and parent and pupil views together, and produces one annual review pack as a PDF, with uploaded reports merged in.

EHC needs assessment requests

For pupils who may need a plan, the EHCNA portfolio guides staff through a structured request to the local authority, pulling in assessment evidence, provision history and the views of the family, and producing the request letter at the end.

Questions schools ask

How often must an EHCP be reviewed?

The SEND Code of Practice requires an EHCP to be reviewed at least every 12 months. ASEND360 tracks review dates and flags reviews that are due.

Can parents contribute to the plan?

Yes. Plans can be shared with parents and carers through the Family Hub, and their views are captured as part of the record.

Can we import existing EHCP outcomes?

Yes. Outcomes can be added from the current plan and then tracked term by term from that point.
